To add parent/guardian information, start by opening the patient profile:

Add a new patient, or click a participant name to open the patient profile.

Scroll down beneath the patient contact information and click +Add Parent/Guardian to expand parent/guardian fields.

Enter information for Parent/Guardian 1. Required fields are marked with an asterisk:
1. Name* – First name*, Middle name, Last name*
2. Relationship* – Options include Father, Mother, Grandparent, Sibling, Parent, Guardian, Other
3. Email Address
4. Phone Number
5. Street
6. Street Line 2
7. City
8. State
9. Zip
To add another parent/guardian, below the parent/guardian, click +Add Parent/Guardian and repeat the process.
To indicate a default parent/guardian, on the right, check Default. You can only select one default parent/guardian. The default parent/guardian will be associated with visits by default, but a different parent/guardian can be selected for individual visits.
To edit parent/guardian information, simply click a field and enter the updated information.
To clear parent/guardian information before saving it, at the right, click the trash can icon.

When you’re finished, scroll to the top right of the profile and click Save Patient.
